This CL
* removes Builtins::codet() and Builtins::codet_handle() returning
  builtins as CodeT objects in favor of code() and code_handle(),
* removes BUILTIN_CODET macro in favor of BUILTIN_CODE,
* removes CodeDataContainer table.
