As the register allocator cannot reuse spill slots, SameAsFirst
constraint means that we would have to do an expensive move to a
differen spill slot if we choose to spill. Forcing the operand
to a register is cheaper.

In zlib, we get >10% speed-up for ia32, >25% for x64.

To manage BasicBlock's predecessors and successors we now use plain
std::vector.

The change also moves bunch of method definitions from header files
to implementation files.

In zlib, the change brings 3x improvement in the scheduler's memory
consumption. The --turbo-stats flag says we go 169MB -> 55MB in
the scheduler, 383MB -> 268MB overall.

- Changes FrameState node to require a 5th input
  (use undefined as sentinel).
- Adds outer_state pointer to FrameStateDescriptor.
- Instruction selection dumps dependencies of FrameState chain into the instruction.
- Code generator builds the translation for rec. FrameStates.

Bunch of changes were necessary:
- refactor attaching the frame states/lazy bailouts in AstGraphBuilder
  (essentialy reland of r23096),
- attaching frame state to some JS nodes in a similar way to attaching
  context (this is quite ugly and we should take another look at this),
- new bailout point for the debugger statement,
- register allocation constraints for the frame states,
- generating translations and deopt entries, attaching them to
  safepoints,
- enabled one mjsunit test for debugger state that uses the generated
  frame state.

MachineType now tracks both the representation and the value type of machine quantities and is used uniformly throughout TurboFan.

These types can now express uint8, int8, uint16, and int16, i.e. signed and unsigned smallish integers. Note that currently only uint8 and uint16 are implemented in the TF backends.
