Too many files know how to deal with decoding, counting, and splitting UTF-8 into uc16 chars. This consolidates several callers who deal with full (Vector<char>, not streaming) bytes by using a UTF-8 Iterator to decode bytes into individual uc16 chars. `WriteUtf16Slow` should not assume that the output buffer has enough bytes to hold both words of surrogate pair. It should pass the number of remaining bytes to the `Utf8::ValueOf` instead, just as we already do in `Utf8DecoderBase::Reset`. Otherwise it will attempt to write the trail uint16_t past the buffer boundary, leading to memory corruption and possible crash. size_t is the correct data type for this purpose. Our APIs (in particular ExternalSourceStream::GetMoreData) are already using it, and there were some static_casts to convert between them. This CL doesn't intend to fix all of V8, just the minimal sense-making part around scanner character streams.
